CONSOMMATION DES PRODUITS PSYCHOATIVES DES ADOLESCENTS DANS LA RÉGION ATSIMO-ANDREFANA ET ANOSY (MADAGASCAR)

Résumé
Introduction
La consommation de substances psychoactives chez les adolescents constitue un problème important de santé publique. Le tabac, l’alcool et le cannabis peuvent avoir des conséquences négatives sur la santé physique, mentale et sociale des jeunes, notamment lorsqu’ils sont consommés précocement. À Madagascar, les données sur ce phénomène restent limitées, particulièrement dans les régions du Sud. Cette étude avait pour objectif d’évaluer la consommation de tabac, d’alcool et de cannabis chez les adolescents des régions Atsimo-Andrefana et Anosy, et d’identifier les facteurs associés.
Méthodes
Une étude épidémiologique observationnelle descriptive à visée analytique a été réalisée en 2024 dans les régions Atsimo-Andrefana et Anosy. Un total de 3 000 adolescents a été inclus, soit 1 500 participants par région. L’échantillonnage a été réalisé selon une méthode aléatoire en deux étapes inspirée des enquêtes internationales HBSC, GSHS et GYTS. Les données ont été recueillies à l’aide d’un questionnaire structuré évaluant les caractéristiques sociodémographiques, la consommation de tabac, d’alcool et de cannabis. Les risques liés à l’alcool et au cannabis ont été évalués respectivement par les scores AUDIT et CAST. Les analyses statistiques ont été réalisées avec le logiciel SPSS, avec un seuil de significativité fixé à p < 0,05.
Résultats
L’âge moyen des participants était de 19,3 ± 3,5 ans. La cohérence interne du questionnaire était satisfaisante avec un alpha de Cronbach de 0,757. La prévalence du tabagisme était de 56,7 % dans la région Atsimo-Andrefana et de 52,3 % dans la région Anosy. La consommation d’alcool concernait 53,3 % des adolescents à Atsimo-Andrefana et 51,2 % à Anosy. La consommation de cannabis était estimée à 22,2 % et 25,5 % respectivement.
La consommation de tabac et d’alcool était significativement plus élevée chez les garçons (p < 0,001). L’âge et le niveau d’instruction étaient également associés à la consommation de substances psychoactives. Parmi les consommateurs d’alcool, 46,6 % à Atsimo-Andrefana et 47,6 % à Anosy présentaient un risque élevé de dépendance selon l’AUDIT.
Conclusion
Cette étude montre une forte consommation de substances psychoactives chez les adolescents des régions Atsimo-Andrefana et Anosy. Le tabac est la substance la plus consommée, suivi de l’alcool et du cannabis. Ces résultats soulignent la nécessité de renforcer les actions de prévention, d’éducation à la santé et de dépistage précoce auprès des adolescents à Madagascar.

Abstract
Introduction:
Psychoactive substance use among adolescents is an important public health issue. Tobacco, alcohol, and cannabis consumption can have harmful effects on young people’s physical, mental, and social health, especially when initiated early. In Madagascar, data on adolescent substance use remain limited, particularly in southern regions. This study aimed to assess tobacco, alcohol, and cannabis use among adolescents in the Atsimo-Andrefana and Anosy regions and to identify associated factors.
Methods
A descriptive analytical observational study was conducted in 2024 in the Atsimo-Andrefana and Anosy regions. A total of 3,000 adolescents were included, with 1,500 participants from each region. A two-stage random sampling method was applied based on international survey approaches (HBSC, GSHS, and GYTS). Data were collected using a structured questionnaire assessing sociodemographic characteristics and psychoactive substance use. Alcohol and cannabis-related risks were evaluated using the AUDIT and CAST scores. Statistical analyses were performed using SPSS software, with statistical significance set at p < 0.05.
Results
The mean age of participants was 19.3 ± 3.5 years. The questionnaire showed satisfactory internal consistency with a Cronbach’s alpha coefficient of 0.757. Tobacco use prevalence was 56.7% in Atsimo-Andrefana and 52.3% in Anosy. Alcohol consumption prevalence was 53.3% and 51.2%, respectively. Cannabis use was reported by 22.2% of adolescents in Atsimo-Andrefana and 25.5% in Anosy.
Tobacco and alcohol consumption were significantly higher among males (p < 0.001). Age and educational level were also associated with psychoactive substance use. Among alcohol users, 46.6% in Atsimo-Andrefana and 47.6% in Anosy showed a high risk of alcohol dependence according to the AUDIT score.
Conclusion
This study highlights a high prevalence of psychoactive substance use among adolescents in the Atsimo-Andrefana and Anosy regions. Tobacco was the most commonly used substance, followed by alcohol and cannabis. These findings emphasize the need to strengthen prevention programs, health education, and early screening strategies among adolescents in Madagascar.
